Many services that we provide are NOT covered by MSP and will be charged to the patient directly. They can be paid in cash, cheque, debit or credit card at the front desk.
As of September 2024, the fees are as follows:
- Doctor’s Note (including Sick Notes): $20 - Insurance Company Form - Short (including records): $164 - Insurance company Form - Long (including records): $215 - Driver’s Exam & Form: $150 - Attending Physician Statement: $180 - Occupational Fitness Assessment: $213 - Chart Transfer (to another clinic or medical practitioner): $43.25 - Physical Fitness Form/Letter (i.e. for camp, school, sports etc): $81.50 - Disability Tax Credit Form: $173 - Drug Benefit Form for Insurance: $89.40 - Rapid Strep Test*: $15 - Travel Consult(s) and Vaccinations: please see our Travel Clinic page
Exceptions and variations to these fees are at the discretion of each physician.
